To recap this week, I focused on finishing up the CDTM literature search and summarizing the results from the identified articles. The results were presented in different formats, including an annotated bibliography table that highlighted key findings, methods, study types, and other relevant info. A brief document the summarized the main findings was also prepared, in addition to a document containing some of the relevant survey and interview questions identified that could be adapted to our study. Overall, the literature review revealed that there are three main barriers to CDTM adoption that need to be addressed: 1) physician perceptions and knowledge of CDTM and willingness to collaborate, 2) regulations and lack of payment mechanisms for pharmacists, and 3) continuity of care between physicians and pharmacists. The first issue remains the biggest hurdle. While many physicians report CDTM as being a high-value service, many lack the knowledge of the scope of practice allowed pharmacists within its framework and may be hesitant to release certain duties. It is important to emphasize that this is a collaborative relationship and not meant to completely take responsibilities away, but rather lighten the workload of physicians by delegating task load, and thereby improving patient care and access to care. Further, more clear guidelines and reimbursement mechanisms are needed so that pharmacists can adequately bill insurance for CDTM services. Many pharmacists may not choose to take on CDTM under the current payment structure because it could potentially add more costs to their practice rather can being cost-effective. Finally, clear expectations are needed in CDTM agreements to ensure continuity of care and streamlined workflow. Ensuring that both physicians and pharmacists have access to relevant patient information while maintaining patient privacy and confidentiality is essential. These findings from the literature review will guide us in the next steps as we begin to prepare survey questions for our study population. The competencies met this week include:
